Tidlig statinbehandling ved akut koronart syndrom

Hos patienter med stabil iskæmisk hjertesygdom kan kolesterolsænkende behandling med 3-hydroxy-3-methylglutaryl-coenzym A-reduktasehæmmer (statiner) reducere morbiditet og mortalitet. I undersøgelser, hvor statinbehandlin- gen er iværksat som sekundær profylakse, er behandlingen påbegyndt 3-6 måneder efter det akutte koronare syndrom, mens det er i den tidlige fase efter et akut myokardieinfarkt eller ustabil angina pectoris, at patienterne har størst risiko for et nyt akut koronart syndrom. Det er ikke dokumenteret, om statinbehandling påbegyndt tidligt efter et akut koronart syndrom kan reducere den tidlige forekomst af koronare hændelser efter det initiale akutte koronare syndrom. Ud over kolesterolreduktionen kan statiner have virkninger på karvæggen. Disse mekanismer kan være bedring af endotelial dysfunktion, plaque -stabilisering, antiinflammatorisk virkning eller plaque -regression. Disse virkninger kan forklare noget af den gavnlige effekt af statinbehandlingen. Det er dog uafklaret, om dette er associeret med en prognostisk effekt ved det akutte koronare syndrom.

Iskæmisk hjertesygdom (IHS) er en hyppig årsag til morbiditet og mortalitet i den vestlige verden (1). I Europa er det estimeret, at cirka en million mennesker per år lider af et akut koronart syndrom (AKS) (2), der dækker over ustabil angina pectoris (UAP), non-ST-elevationsmyokardieinfarkt og ST-elevationsmyokardieinfarkt. Hos patienter med stabil IHS (3-5) eller risikofaktorer over for IHS (6-7) kan farmakologisk kolesterolreducerende behandling reducere langtidsmortalitet og -morbiditet. I disse undersøgelser påbegyndte patienterne statinbehandling 3-6 måneder efter et AKS. Imidlertid er der i den tidlige fase efter et AKS en højere risiko for sekundære komplikationer sammenlignet med patienter, der har stabil IHS. Da statinbehandling er associeret med en gunstig effekt på endotelet, er der derfor en teoretisk baggrund for at påbegynde statinbehandling tidligt ved AKS for at forbedre prognosen.

Studier med AKS og tidlig statinbehandling

Relevante artikler er fundet ved søgning på MEDLINE med brug af søgeordene: acute coronary syndrome, acute myocardial infarction, early statin treatment, aterovastatin, cerivastatin, fluvastatin, simvastatin, pravastatin og statin samt i abstractbøger fra europæiske og amerikanske kardiologiske kongresser (år 1999, 2000 og 2001). Der blev fundet syv studier (Tabel 1 ), der vurderede effekten af tidlig statinbehandling efter AKS, fem af studierne var randomiserede undersøgelser: MIRACL (8), FLORIDA (9), RECIFE (10), PTT (11) og L-CAD (12), og to studier var observationelle studier: Swedish Register of Cardiac Intensive Care (13), og et studie fra GUSTO IIb (14) og PURSUIT (15) studierne, hvor den samlede mortalitet blandt patienter med AKS blev sammenlignet mellem patienter, der fik et statinpræparat ved udskrivelsen (n=3.653), og patienter, der ikke var i statinbehandling ved udskrivelsen (n=17.156) (16).

I MIRACL-studiet (8) indgik 3.086 patienter med UAP eller non-ST-elevationsmyokardieinfarkt, som blev randomiseret til atorvastatin 80 mg dagligt eller placebo. Behandlingen blev påbegyndt inden for 24-96 timer efter et AKS, og patienterne blev fulgt i 16 uger. Det primære kombinerede effektmål død, hjertestop, AMI eller UAP, der krævede hospitalisering, forekom hos 17,4% i placebogruppen og hos 14,8% i atorvastatingruppen, en relativ risikoreduktion på 16% med et konfidensinterval på 0%-30%; p=0,048 (relativ risiko 0,84; 95% konfidensinterval 0,70-1,00; p=0,048). Der var ikke nogen signifikant forskel i død, nonletalt AMI eller hjertestop mellem de to grupper. Atorvastatin påvirkede ikke den hurtige forekomst af koronare hændelser, der forekom inden for de første fem uger efter det initiale AKS, hvilket udgjorde 70% af samtlige hændelser i de 16 uger.

L-CAD-studiet (12) viste ligeledes reduktion af koronare hændelser og regression af koronaraterosklerotiske læsioner vurderet ved koronarangiografi.

Flere undersøgelser pågår for at vurdere effekten og sikkerheden ved tidlig statinbehandling: Australian PACT-studiet sammenligner pravastatin 40 mg dagligt med placebo, hvor behandlingen påbegyndes inden for 24 timer efter et AKS. PROVE-IT-studiet sammenligner effekten af pravastatin 40 mg dagligt mod atorvastatin 80 mg dagligt, hvor behandlingen påbegyndes inden for ti dage efter et AKS. Patienterne i begge statingrupper randomiseres endvidere til et antibiotikum eller placebo. A to Z-studiet undersøger effekten af simvastatin på iskæmiske hændelser efter et non-ST-elevations AKS. PRINCESS-studiet undersøger effekten af cerivastatin som forebyggelse af reinfarkt.

Mulige mekanismer

Statiner har virkninger på karvæggen, der ikke udelukkende skyldes lipidreduktionen. Disse virkninger kan forklare noget af den gavnlige effekt af statinbehandlingen (17-21).

Endoteldysfunktion

Endotelet frigiver vasokontraherende og vasodilaterende faktorer, der er med til at regulere kartonus og myokardieperfusionen, og det er med til at opretholde en antitrombogen overflade i karrene, da det inhiberer adhærens af trombocytter og leukocytter til karvæggen. I et intakt endotel foregår der er lokal produktion af nitrogenoxid (NO) fra aminosyren arginin ved hjælp af enzymet nitrogen-oxid-syntase (eNOS). NO aktiverer en enzymatisk kaskade, der fører til en vasodilatation. Ved aterosklerose er den endotelafhængige vasodilatation svækket i koronarkarrene på grund af en nedsat frigivelse eller aktivitet af NO (22). En svækket produktion og aktivitet af NO kan også ledsage hyperkolesterolæmi (23, 24), og en øget koncentration af oxideret LDL-kolesterol kan måske direkte inhibere NO ved en alt for stor produktion af oxygen-deriverede frie radikaler og reduceret transkription eller øget posttranslateret destabilisering af eNOS-mRNA.

En af mekanismerne af statiners gavnlige effekter på endotelet kan være en NO-afhængig mekanisme, hvor statiner kan medføre en opregulering af e-NOS-aktiviteten, der bevarer endotelets NO-syntese (25).

Flere undersøgelser har vist, at den endoteliale dysfunktion er delvist reversibel inden for kort tid efter påbegyndt statinbehandling (14, 26, 27).



Plaquestabilisering

Stabiliteten af et aterosklerotisk plaque er bestemt af en kontinuerlig, dynamisk balance mellem påvirkning af inflammatoriske celler, der tenderer mod at destabilisere et plaque , og de glatte muskelcellers reparative egenskaber, der kan stabilisere et plaque . Ustabile eller vulnerable plaques har en stor trombogen lipidrig kerne, omgivet af en tynd fibrøs kappe der består af glatte muskelceller og inflammatoriske celler, som overvejende er makrofager. De glatte muskelceller er den eneste type celler, der er i stand til at syntetisere en stærk fibrøs kappe, da de kan producere ekstracellulære matrixproteiner som kollagen. Tilstedeværelsen af glatte muskelceller er derfor vigtig for plaque -stabiliteten, mens aktiverede inflammatoriske celler øger risikoen for plaque - ruptur.

Subendotelial akkumulering af oxideret LDL kan tiltrække makrofager og stimulere deres binding til endotelet med adhæsionsmolekyler. Aktiverede makrofager frigiver matrix-metalloproteinaser (MMP), der kan ødelægge matrixproteiner i den fibrøse kappe (28), mens aktiverede T-celler kan frigive interferon-γ, der kan inhibere syntese af glatte muskelcellers matrixkollagen (29, 30).

Pravastatinbehandling kan reducere lipidoxidation, inflammation, MMP-2 og celledød, og øge tissue inhibitor for metalloproteinase 1 og kollagen-plaque fra humane carotis-arterier (31). Virkningen af statiner går i retning af at stabilisere et plaque, idet det ændres, så det indeholder færre inflammatoriske celler og flere glatte muskelceller og en øget mængde kollagen.

Antiinflammatorisk effekt

C-reaktivt protein (CRP) er en sensitiv markør for en underliggende systemisk inflammation, og forhøjet CRP er også forbundet med en øget risiko for iskæmisk hjertesygdom. Statinbehandling kan ud over en reduktion i kolesterol også forårsage en reduktion i CRP, og reduktionen ses både hos patienter med iskæmisk hjertesygdom og hos personer med en øget risiko for iskæmisk hjertesygdom (32-34). Hvis inflammatoriske markører som CRP og S-amyloid A er forhøjet efter et myokardieinfarkt, har en undersøgelse vist, at det kan være forbundet med en øget risiko for en ny kardial event (35). En af statinernes virkninger kan være en svækkelse af den inflammatoriske indflydelse på risikoen for udvikling af koronare events.

Plaqueregression

Generelt har der været en beskeden sammenhæng mellem ændringen i de koronarangiografiske forandringer og ændringen i lipider, men der har ikke været nogen sammenhæng mellem ændringen i de angiografiske forandringer og de kliniske hændelser (36-39).

Konklusion

Om statinbehandling i den tidlige fase efter et AKS kan reducere forekomsten af tidlige kardiale komplikationer som død, AMI og UAP er ikke afklaret i dag. Der foreligger en enkelt randomiseret undersøgelse (8), hvor der ikke fandtes nogen signifikant forskel i død, nonletalt AMI eller hjertestop mellem de to grupper. Der er dog en gunstig effekt på surrogatparametre, der har været associeret med en prognostisk gevinst, og flere undersøgelser pågår for at undersøge dette. Et andet aspekt er, at statinbehandling, der påbegyndes under indlæggelsen ved den initiale koronare event, formentlig vil øge kompliansen for behandlingen (40) og kan medføre, at en større andel af patienter, der har behov for en relevant og prognostisk betydende, farmakologisk kolesterolsænkende behandling, vil modtage denne behandling.

Lipid lowering treatment with 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitor (statins) may reduce morbidity and mortality in patients with stable ischaemic heart disease. In trials where a statin was used as secondary prevention after an acute coronary syndrome (ACS), the treatment was initiated 3-6 months after the event. A high rate of serious complications occurs in the unstable clinical state after an ACS, including myocardial infarction and death. The incidence rate of serious complications declines after 1 month and then gradually diminishes. It has not been determined whether initiation of treatment with a statin soon after an ACS reduces the occurrence of early coronary events. Statins have effects on the vessel wall other than the lipid lowering effect, and these may partly account for the benefit of statin therapy. Whether these mechanisms are associated with a prognostic effect soon after an ACS has not been clarified.
